About Case Management Services

Case management can help when you are trying to navigate services, resources, systems, or practical challenges and are not sure where to begin.

Working under ongoing supervision and direction, our Social Service Worker student can provide support with:

  • Advocacy and assistance navigating barriers to services
  • Developing practical goals and action plans
  • Identifying additional supports when needs fall outside the scope of case management services
  • Identifying and accessing relevant programs and services
  • Identifying needs, strengths, goals, and available resources
  • Navigating community and social service systems
  • Referrals and connections to community resources
  • Resource and information gathering
  • Service coordination and follow-up
  • Supporting communication and coordination with appropriate service providers

Case management is collaborative and focused on helping you identify and access supports that are appropriate to your circumstances and goals.

Important Information About This Service

These services are provided by a Social Service Worker student completing the Social Service Worker diploma program at Loyalist College as part of their supervised field placement.

Case management services are not counselling or psychotherapy. The student does not provide psychotherapy, psychological treatment, diagnosis, or independent clinical assessment.

The student works within the scope of their education, training, experience, and demonstrated competence and receives ongoing supervision and direction. Where an individual's needs require services beyond the student's role or competence, appropriate consultation, referral, or additional support will be identified.

Who May Benefit From Case Management?

Case management may be appropriate for individuals who:

  • Are experiencing barriers to accessing services
  • Are having difficulty navigating health, mental health, social, housing, financial, employment, or community services
  • Are unsure what resources or supports may be available to them
  • Need help connecting with community organizations or other service providers
  • Want assistance identifying practical next steps toward a personal or social goal
  • Would benefit from assistance coordinating multiple services or supports

Things We Cannot Assist With

Case management services are not intended to provide:

  • Counselling or psychotherapy
  • Crisis intervention or emergency mental health services
  • Diagnosis or treatment of mental health disorders
  • Independent risk assessment or management
  • Services requiring expertise or authority beyond the student's education, training, or supervised role
  • Specialized clinical assessment or treatment

If you are experiencing an immediate mental health crisis or are at risk of harming yourself or someone else, please contact 911 or 988 rather than submitting a case management request.

Supervision and Professional Development

As part of the student's field placement, case management activities may be discussed with the student's placement and clinical supervisors for educational, supervisory, and quality-assurance purposes.

Information will be handled in accordance with applicable privacy and confidentiality requirements and the policies of Adam Terpstra Psychotherapy Professional Corporation and Bridging With Support.

Service Fee

Student Case Management Services: $65 per hour

The service fee is for case management and social service support and is separate from the counselling and psychotherapy services offered by our registered mental health professionals.

Case management services may not be eligible for reimbursement through extended health insurance plans. We recommend contacting your insurer directly if you wish to determine whether your plan provides coverage for social service or case management services.

Before You Submit a Request

Please understand that submitting a request does not guarantee that case management services will be appropriate for your circumstances or that services can be provided.

Following submission, we will review your request and determine whether the student's role, available supervision, and available resources are appropriate to your needs. Where another service or provider would be more appropriate, we will provide information about potential next steps where possible.
